Importance Informed consent is essential to ethical, rigorous research and is important to recruitment and retention in cancer trials.Objective To examine cancer clinical trial (CCT) participants’ perceptions of informed consent processes and variations in perceptions by cancer type.Design and Setting and Participants Cross-sectional survey from mixed-methods study at National Cancer Institute–designated Northeast comprehensive cancer center. Open-ended and forced-choice items addressed: (1) enrollment and informed consent experiences and (2) decision-making processes, including risk-benefit assessment. Eligibility: CCT participant with gastro-intestinal or genitourinary, hematologic-lymphatic (...) malignancies, lung cancer, and breast or gynecological cancer (N = 334).Main Outcome Measures Percentages satisfied with consent process and information provided; and assessing participation’s perceptions of risks/benefits. Multivariable logistic or ordinal regression examined differences by cancer type.Results Most patient-participants felt well informed by the consent process (more than 90% overall and by cancer type) and. most (87.4%) reported that the consent form provided all the information they wanted, although nearly half (44.8%) reported that they read the form somewhat carefully or less. More than half (57.9%) said that talking to research staff (i.e., the consent process) had a greater impact on participation decisions than reading the consent form (2.1%). A third (31.1%) were very sure of joining in research studies before the informed consent process (almost half of lung cancer patients did—47.1%). Most patients personally assessed the risks and benefits before consenting. However, trust in physicians played an important role in the decision to enroll in CCT.Conclusions and Relevance Cancer patients rely less on written features of the informed consent process than on information obtained from the research staff and their own physicians. Research should focus on information and communication strategies that support informed consent from referring physicians, researchers, and others to improve patient risk-benefit assessment and decision-making. The economical/environmental scheduling problem of the ship integrated energy system has high computational complexity, which includes more than one optimization objective, various types of constraints, and frequently fluctuated load demand. Therefore, the intelligent scheduling strategies cannot be applied to the ship energy management system online, which has limited computing power and storage space. Aiming at realizing green computing on SEMS, in this paper a typical SIES-EESP optimization model is built, considering the form of decision vectors, the economical/environmental optimization objectives, and (...) various types of real-world constraints of the SIES. Based on the complexity of SIES-EESPs, a two-stage offline-to-online multiobjective optimization strategy for SIES-EESP is proposed, which transfers part of the energy dispatch online computing task to the offline high-performance computer systems. The specific constraints handling methods are designed to reduce both continuous and discrete constraints violations of SIES-EESPs. Then, an establishment method of energy scheduling scheme-base is proposed. By using the big data offline, the economical/environmental scheduling solutions of a typical year can be obtained and stored with more computing resources and operation time on land. Thereafter, a short-term multiobjective offline-to-online optimization approach by SEMS is considered, with the application of multiobjective evolutionary algorithm and typical schemes corresponding to the actual SIES-EESPs. Simulation results show that the proposed strategy can obtain enough feasible Pareto solutions in a shorter time and get well-distributed Pareto sets with better convergence performance, which can well adapt to the features of real-world SIES-EESPs and save plenty of operation time and storage space for the SEMS. In this essay I argue that what has been called “philosophical anarchism” in the academic literature bears little to no relationship with the historical anarchist tradition and, for this reason, ought not to be considered a genuine form of anarchism. As I will demonstrate, the classical anarchism of the nineteenth and early twentieth centuries is to be distinguished from other political theories in regarding all hierarchical institutions and relationships—including, but not limited to, the state—as incorrigibly dominative or oppressive and, for (...) this reason, immoral. Lastly, I argue that defenders of such institutions and relationships must take the challenge posed by classical anarchism seriously by engaging substantively with actual anarchist positions. ObjectivesWuhan is the city where coronavirus disease was first reported and developed into a pandemic. However, the impact of the prolonged COVID-19 pandemic on medical staff burnout remains limited. We aimed to identify the prevalence and major determinants of burnout among medical staff 1 year after the beginning of the COVID-19 pandemic in Wuhan, China.Materials and MethodsA total of 1,602 medical staff from three hospitals in Wuhan, China, were included from November 1–28, 2021. Chi-square tests were conducted to compare the (...) prevalence of burnout across groups based on sociodemographic and professional characteristics. A multivariate analysis was performed using a forward stepwise logistic regression model.ResultsApproximately 37.39% of the medical staff experienced burnout 1 year after COVID-19 pandemic. Emotional exhaustion was the most common symptom of burnout, with 1,422 participants reporting a severe EE. Burnout was associated with insufficient social support and “neutral” or “dissatisfied” patient-physician relationships. Respondents who participated in the care of COVID-19 patients had a higher risk of burnout symptoms than those who did not participate. In particular, mental resilience was negatively associated with burnout among the medical staff.ConclusionNearly two-fifths of the participants had symptoms of burnout, with reduced personal accomplishment being the predominant symptom 1 year after COVID-19. Healthcare organizations should regularly measure and monitor burnout among the medical staff. In addition, creating positive work environments and improving the mental resilience of medical staff may be effective ways to reduce burnout. (shrink)

Current computational models predict reward based solely on learning. Real motivation involves that but also more. Brain reward systems can dynamically generate incentive salience, by integrating prior learned values with even novel physiological states (e.g., natural appetites; drug-induced mesolimbic sensitization) to cause intense desires that were themselves never learned. We hope future computational models may capture this too.

Language is central to the interactional nature of the social life within which it is situated. To react or respond in a particular situation, we must be able to recognize the social situation. Growing evidence has demonstrated the negative impact of perceived loneliness on late-life executive functions. Yet little is known about how social factors impact language processing for older people. The current study aims to fill this gap, first by assessing age-related changes in lexical processing during Chinese word reading, (...) second, by examining whether older adults’ individual differences, such as processing speed and verbal abilities, modulate meaning retrieval and, third, by investigating whether perceived loneliness can hinder word reading. The use of compound words in Chinese enables significant sublexical ambiguity, requiring varying executive load during word recognition: when a word’s constituent characters carry multiple meanings, readers must consider the meaning contributions of both constituent characters and use top-down word information to determine the most accurate meaning of the ambiguous character, a process termed “sublexical ambiguity resolution.” In this study, adults read real Chinese words and pseudowords, and they were asked to make lexical decisions. Older adults exhibited greater lexicality effects and similar sublexical ambiguity effects compared with young adults. Among older participants, processing speed could account for their ability to differentiate between words and pseudowords. In contrast, the level of perceived loneliness modulated the efficacy of sublexical ambiguity resolution: the participants with higher perceived loneliness displayed a greater sublexical ambiguity disadvantage effect. These results indicate that perceived loneliness may affect the use of contextual information in meaning retrieval during reading. Characterizations of length neutrosophic subalgebras are discussed by using level sets of interval neutrosophic sets. Conditions for level sets of interval neutrosophic sets to be subalgebras are provided.

Aworkshop was held August 26–28, 2015, by the Earth- Life Science Institute (ELSI) Origins Network (EON, see Appendix I) at the Tokyo Institute of Technology. This meeting gathered a diverse group of around 40 scholars researching the origins of life (OoL) from various perspectives with the intent to find common ground, identify key questions and investigations for progress, and guide EON by suggesting a roadmap of activities. Specific challenges that the attendees were encouraged to address included the following: What key (...) questions, ideas, and investigations should the OoL research community address in the near and long term? How can this community better organize itself and prioritize its efforts? What roles can particular subfields play, and what can ELSI and EON do to facilitate research progress? (See also Appendix II.) The present document is a product of that workshop; a white paper that serves as a record of the discussion that took place and a guide and stimulus to the solution of the most urgent and important issues in the study of the OoL. This paper is not intended to be comprehensive or a balanced representation of the opinions of the entire OoL research community. It is intended to present a number of important position statements that contain many aspirational goals and suggestions as to how progress can be made in understanding the OoL. The key role played in the field by current societies and recurring meetings over the past many decades is fully acknowledged, including the International Society for the Study of the Origin of Life (ISSOL) and its official journal Origins of Life and Evolution of Biospheres, as well as the International Society for Artificial Life (ISAL). Objective: The objectives of this study are to understand the current functions, structure and operation of hospital ethics committees (HECs) in Shanghai and to facilitate their improvement. Methods: (1) A questionnaire survey, (2) interviews with secretaries and (3) on-site document reviews of HECs in Shanghai were used in the study, which surveyed 33 hospitals. Results: In Shanghai, 57.56% of the surveyed hospitals established HECs from 1998 to 2005. Most HECs used bioethical review of research involving human subjects as well as (...) bioethical review or consultation regarding medical care services and administrative decision- making. Of the surveyed HECs, 14.3% did not provide any formal bioethical training to the HECs’ members and many HECs had no standard operating procedures. Some HECs had no clear definition of what was “conflict of interest” that should be considered by the HECs, while 44.4% of the HECs did not perform continuing review. Discussion: After the issues of related national regulations, more and more hospitals established HECs in Shanghai, but the functions of HECs need to be further developed and formal training on bioethics should be provided to HEC members. To assure the independence and good performance of HECs, the conflict of interest procedure, the standard operating procedures and bioethical review should be improved. Conclusion: HECs in Shanghai had developed in the preceding 10 years and they played great roles in protecting the rights and welfare of human subjects and patients; some areas need improvement. The rise of global civil society as a new discourse in the last century, in the new century has been of concern. Western academic circles around the concept of global civil society, global civil society and the emergence and development of global civil society influence on the political realities and trends and other issues had extensive discussions and the formation of a different theoretical framework and research camp. While the Western world made ​​a series of civil society, research results, but (...) still insufficient to demonstrate the normative study, mainly confined to the liberal democratic framework of the inadequacies of the middle. China scholars have been from international relations, political science and political philosophy point of view about global civil society, but in general, the power of research in this area is still relatively weak, insufficient results, further research needs to be made. As a word rising from the late of last century, Global Civil Society is very attractable in the early new century. Western academy has initiated abroad discussion on the concept of GCS, the process of its formation and development, its impact on political realities and its future. In the discussion, theory analysis frames and theory camps have already come into being. Though a great achievements have been gotten, there are still some deficiencies lie in the Western studies, such as lacking of normative studies and main studies are limited in the frames of liberal democracy. In China some scholars have studied about GCS from the views of international relationship, political science and political philosophy. But as a whole, research force in the realm is still weak and the fruits are insufficient, deep discussion is needed. (shrink)

In three decades of reform China has become a society that is radically different both from what it used to be in Mao's time and from a liberal society. This new China poses especially interesting questions about freedom - interesting not only in the Chinese context but also more generally. Pivotal for my treatment of these questions is a distinction I draw between de facto freedom and the value of freedom, the latter in turn understood in terms of a larger (...) notion of agency, and divided into such distinct dimensions as moral resource, right, and mode of subjection. This distinction allows me to argue that while China has no shortage of de facto freedom, it lacks the value of freedom, especially freedom as a mode of subjection. The lack of freedom as a value has serious moral and political consequences, as manifested in a prolonged moral crisis in post-Mao China and in a certain fragility of social order caused by the pitting of order against freedom. Such consequences suggest a strong need for China to develop the value of freedom. But it is possible and important to do so in a critical spirit, as I argue through a critical analysis of the place of freedom in a liberal society. © 2011 by Duke University Press. (shrink)

[Review] Liz P.Y. Chee. Mao’s Bestiary: Medicinal Animals and Modern China. Duke University Press, 2021. 288 pp. The COVID-19 pandemic has secured its place as a 21st century global public health disaster. It has killed more than 6.2 million and infected close to 500 million people worldwide. Acknowledging Wuhan’s wildlife market as the ground zero of the pandemic and the devastation caused by SARS 17 years earlier, China’s Communist authorities made the long overdue decision on February 24, 2020 and outlawed (...) wildlife breeding and trade for the country’s exotic food market. This decision was commendable. Yet, breeding of wildlife for the exotic food market was only one of the five-piece captive farming operation that generated a revenue of $78 billion a year. What the Chinese authorities have retained is captive breeding for Traditional Chinese Medicine, the third largest component of the country’s controversial industry. Called a ‘national treasure’, TCM, in the minds of many, brooks no questioning. Liz P.Y. Chee’s Mao’s Bestiary: Medicinal Animals and Modern China steps in this minefield with questions not about the efficacy of TCM but about the drivers of its faunal medicalization in the last seven decades. Modern Chinese history told from a Buddhist perspective restores the vibrant, creative role of religion in postimperial China. It shows how urban Buddhist elites jockeyed for cultural dominance in the early Republican era, how Buddhist intellectuals reckoned with science, and how Buddhist media contributed to modern print cultures. It recognizes the political importance of sacred Buddhist relics and the complex processes through which Buddhists participated in and experienced religious suppression under Communist rule. Today, urban and rural communities alike engage with (...) Buddhist practices to renegotiate class, gender, and kinship relations in post-Mao China. Using fresh archival and primary sources, along with extensive ethnographic research, this volume vividly portrays these events and more, recasting Buddhism as a critical factor in China's twentieth-century development. Each chapter connects a moment in Buddhist history to a significant theme in Chinese history, creating new narratives of Buddhism's involvement in the emergence of urban modernity, the practice of international diplomacy, the mobilization for total war, and other transformations of state, society, and culture. The transcriptional co‐activators YAP (or YAP1) and TAZ (or WWTR1) are frequently activated during the growth and progression of many solid tumors, including lung, colorectal, breast, pancreatic, and liver carcinomas as well as melanoma and glioma. YAP/TAZ bind to TEAD‐family co‐activators to drive cancer cell survival, proliferation, invasive migration, and metastasis. YAP/TAZ activation may also confer resistance to chemotherapy, radiotherapy, or immunotherapy. YAP‐TEAD cooperates with the RAS‐induced AP‐1 (FOS/JUN) transcription factor to drive tumor growth and cooperates with MRTF‐SRF to promote (...) activation of cancer‐associated fibroblasts, matrix stiffening, and metastasis. The key upstream repressor of YAP/TAZ activation is the Hippo (MST1/2‐LATS1/2) pathway and the key upstream activators are mechanically induced Integrin‐SRC and E‐cadherin‐AJUBA/TRIP6/LIMD1, growth factor induced PI3K‐AKT, and inflammation‐induced G‐protein coupled receptor (GPCR) signals, all of which antagonize the Hippo pathway. In this review, strategies to target YAP/TAZ activity in cancer are discussed along with the prospects for synergy with established pillars of cancer therapy. (shrink)

Heterogeneity within MDD has hampered identification of biological markers (e.g., intermediate phenotypes, IPs) that might increase risk for the disorder or reflect closer links to the genes underlying the disease process. The newer characterizations of dimensions of MDD within Research Domain Criteria (RDoC) domains may align well with the goal of defining IPs. We compare a sample of 25 individuals with MDD compared to 29 age and education matched controls in multimodal assessment. The multimodal RDoC assessment included the primary IP (...) biomarker, positron emission tomography (PET) with a selective radiotracer for 5-HT1A ([11C]WAY-100635), as well as event-related functional MRI with a Go/No-go task targeting the Cognitive Control network, neuropsychological assessment of affective perception, negative memory bias and Cognitive Control domains. There was also an exploratory genetic analysis with the serotonin transporter (5-HTTLPR) and monamine oxidase A (MAO-A) genes. In regression analyses, lower 5-HT1A binding potential (BP) in the MDD group was related to diminished engagement of the Cognitive Control network, slowed resolution of interfering cognitive stimuli, one element of Cognitive Control. In contrast, higher/normative levels of 5-HT1A BP in MDD (only) was related to a substantial memory bias toward negative information, but intact resolution of interfering cognitive stimuli and greater engagement of Cognitive Control circuitry. The serotonin transporter risk allele was associated with lower 1a BP and the corresponding imaging and cognitive IPs in MDD. Lowered 5HT 1a BP was present in half of the MDD group relative to the control group. Lowered 5HT 1a BP may represent a subtype including decreased engagement of Cognitive Control network and impaired resolution of interfering cognitive stimuli. Future investigations might link lowered 1a BP to neurobiological pathways and markers, as well as probing subtype-specific treatment targets. (shrink)

In this report, we describe the case of a 34-year-old man who developed abnormal liver tests 6 years after live-donor kidney transplantation and 5 years after initiation of sirolimus-based immunosuppression. Elevated aminotransferase levels persisted after withdrawal of potentially hepatotoxic medications, and serologic evaluation for viral hepatitis, autoimmune disease, and genetic disorders was unrevealing. Liver biopsy revealed prominent hepatocellular injury associated with a mixed inflammatory infiltrate and liver tests normalized within 2 weeks of discontinuation of sirolimus. In this report, we review previous reports of sirolimus hepatotoxicity and propose a unifying hypothesis for the various inflammatory conditions that have been attributed to sirolimus.Keywords: sirolimus, immunosuppression, transplant, inflammatory conditions, hepatotoxicity. On this basis, that: exaggerate the impact of traditional culture of Mao Zedong, Mao Zedong's philosophy of originality exaggerated and its Marxist philosophy dissimilarity between, in (...) fact, is a fragmented Marxism-Leninism Mao Zedong Thought, to the historical development, see also Marxism is not a continuous open system, which also reduces Marxism in China's cultural significance, does not help us to correctly understand and grasp the philosophy of Mao Zedong's cultural identity. In response to some western Maoist scholars who overstate the role of traditional Chinese cultural ingredients in the ideology of Mao and attribute the development of Marxism in China totally to the process of "Confucianization", the essay aims at revealing the cultural characteristics of Mao's philosophy by way of a detailed analysis of Mao's indebtedness to and innovation of traditional Chinese philosophy. Furthermore, the essay holds that various overstatements of the role that traditional Chinese culture has played in Mao Ze-dong's idea and of the discrepancy between Maoist philosophy and Marxist philosophy mean to cut off the connection between Marx-Leninism and Mao's thought. In consequence they overlook the openness of Marxism, and degrade the significance of the development of Marxism in China and also prevent us from understanding the dual influence of Mao's philosophy.
